East Employees

As an associate in the East region, you are responsible for calling your supervisor/absence administrator on the first day of absence.

You are also responsible for calling Met-Life at 1-888-608-6665 or www.metlife.com/mybenefits, Fax# 800-230-9531, should the absence period continue beyond seven calendar days.

This is required in order to apply for Short Term Disability benefits. The call should be made to MetLife on or before the eighth calendar day from your first day of absence. In addition, if you return to work from a disability absence and are then absent again for any reason within 14 calendar days, on the first day of this subsequent absence you must call your supervisor/absence administrator and immediately call MetLife at 1-888-608-6665. Call in to your Absence Administrator 1st day.

What Is a Short Term Disability (STD) Benefit Plan?

A benefit plan that replaces a portion of income lost because of an injury or illness that prevents an employee from working. Benefits may continue for a certain length of time as specified by the particular plan. For example, 1 week, 13 weeks, 26 weeks, 52 weeks, or until Long Term Disability benefits become payable. Please reference your plan materials to determine how long your benefits may continue.

What Is a Long Term Disability (LTD) Benefit Plan?

A benefit plan that replaces a portion of income lost because of an injury or illness that lasts for an extended period of time and prevents an employee from working. Benefits may be paid following an elimination or waiting period and may continue to the maximum length of time specified by the plan; for example, until the employee reaches a certain age. Please reference your plan materials to determine how long your benefits may continue.

What Is the Family/Medical Leave Act (FMLA)?

FMLA stands for the Family/Medical Leave Act, a federal law enacted in 1993. Under the FMLA, eligible employees may take up to 12 weeks time off from work because of their own serious health condition, or to care for a spouse, child or parent who has a serious health condition, or to care for a newborn child, newly adopted child, or newly placed foster child, or any qualifying exigency arising out of the fact that the spouse, son, daughter, or parent of the employee is on active duty. Leave may be taken in continuous spans of time or through intermittent or reduced-schedule leave, subject to certain exceptions and limitations. Please contact your employer to determine how FMLA is administered at your company.

Workers’ Compensation Claims

As of 1/1/09, Verizon New York no longer self-insures its Workers Compensation claims.

AIG is now the Workers’ Compensation carrier for Verizon New York.

The claims are processed by Sedgwick Claims Management. When you are injured “on the job” you will receive a packet of forms from Sedgwick.  The packet contains a number of forms, questionnaires and medical releases.

With the exception of the C3 (and possibly the C3.3) form, (which Fine, Olin and Anderman prefer to complete and file for members that retain them) NONE of the releases or questionnaires should be completed. By signing some of the releases the injured Member is giving Sedgwick access to their medical records.

In addition, by signing the C3.1 form, an injured Member is giving Verizon the authority to select their treating doctor.

There is no requirement in the law for an injured worker to give such authority to the employer and we strongly recommend against it.

After an “on the job” accident:

  1. Seek  medical attention, if needed, immediately.
  2. Notify your supervisor
  3. Contact a lawyer, FOA (Fine, Olin and Anderman) at 1-800-522-9001.
